Medicaid In-Home Health Care
We provide Medicaid-approved home health care services to eligible individuals, ensuring they receive care, support, and medical assistance to live safely at home.
Contact Us- Must be a Missouri Medicaid (MO HealthNet) recipient
- Must require assistance with daily living activities as determined by an assessment
- Must have a physician’s referral for home health care services
Consumer Directed Services (CDS)
With CDS, you are in control of your care. This program allows eligible Medicaid recipients to hire and manage their own caregivers, including family members, to assist with daily living activities.
Contact Us- Must be 18 years or older
- Must be on Missouri Medicaid (MO HealthNet)
- Must have a physical disability requiring assistance with daily activities
- Must be able to self-direct their care or have an authorized representative
MO HealthNet for Disabled Children
MO HealthNet provides essential medical and in-home care services for disabled children, ensuring they receive the best possible care in a safe and comfortable environment.
- Child must be under age 18
- Must be Medicaid (MO HealthNet) eligible
- Must have a qualifying disability or medical condition
Healthy Children & Youth Program (HCY)
Through the HCY program, we offer preventative and specialized healthcare services to children and youth, ensuring they receive early intervention, screenings, and continuous care to support their growth and development.
- Child must be under age 18
- Must be enrolled in MO HealthNet (Medicaid) for Kids
- Must meet medical criteria for early intervention services
Veterans Home Care Program
For individuals who do not qualify for Medicaid or prefer customized home care solutions, we offer affordable private pay options. Our services can be tailored to fit specific needs, from basic daily support to advanced medical care.

- Must be eligible for VA Aid & Attendance benefits
- Must require assistance with activities of daily living (ADLs)
Private Pay Home Care
